Loan Applicants Get a head start on the loan process by clearing some paperwork out of the way. Each application and form provided here is a "PDF" file to be viewed with Adobe Acrobat Reader. If you don't have the software already, click here to download it for free... 1. CLICK HERE FOR OUR LOAN APPLICATION. It will open up automatically in Acrobat Reader, and you can fill-in all the information right on your screen. The software will even make all the necessary calculations for you on the spot! When you're through, print out each of the four pages of the completed application (it's a larger, legal-sized 8.5"X14" form, so you'll need to use paper that size... make sure you specify the correct paper size in your printer options, too). You can mail the document to me or give it to me directly next time we meet for fast processing. 2. CLICK HERE FOR OUR AUTHORIZATION FORM. After it opens automatically in Acrobat Reader, print the single-page form from your computer (it's a larger, legal-sized 8.5"X14" form, so you'll need to use paper that size... make sure you specify the correct paper size in your printer options, too). Have each potential borrower carefully read and initial every individual section and sign and date the bottom of the form. You can mail the document to me or give it to me directly next time we meet for fast processing. 3.CLICK HERE FOR OUR CONSENT FORM. After it opens automatically in Acrobat Reader, print the single-page form from your computer (it's a standard, letter-sized 8.5"X11" form, so regular printer paper will work). Have each potential borrower carefully read the form in its entirety and provide their signature, date and social security number.
